Documenting the Clinical Need for Leave
A psychiatric evaluation may address the effect of symptoms on essential work functions, the expected duration of limitations, and the need for treatment or follow-up.
For eligible employees of covered employers, FMLA may provide job-protected leave for a qualifying serious mental health condition. Employers may request medical certification. Eligibility and leave decisions are separate from the clinical assessment.

- A job description or summary of essential duties
- Relevant treatment history and current medications
- Information about the requested leave schedule
